Our Salon Policy
South Bay Dog Grooming (herein referred to as SBDG) wants to make your experience a pleasant one. We will do all we can to make your pet feel comfortable and will advise you of any abnormalities seen while grooming your pet. If you are not satisfied with your pet’s grooming, we will do all we can to rectify the problem such as: re-bathing, re-cutting, re-brushing or give you a limited credit toward your next appointment. However, we will not give refunds.
PET GROOMING LIABILITY WAIVER AND RELEASE FORM
Initial
1. Health & Behavior of Pet: I agree and understand the SBDG has relied upon my representation that my pet is in good health, has not injured or shown threatening behavior to any persons or animals, is current on all required vaccinations and is free of parasites.
2. Aggressive Pets/Parasites. SBDG reserves the right to refuse service, or stop a groom in progress. If your pet poses a threat to themselves, other pets or staff, whether it be an aggression problem, health problem, or parasite problem.
3. Owner Responsibility: The owner agrees to be solely responsible and liable for any and all acts of behavior of their pet. This may include but is not limited to injury or death to a pet, injury or death to a staff member or any other member of the public. Medical treatment expenses required by a staff member, a member of the public or for another pet will be the sole responsibility of the pet owner.
4. Matted Hair on Pet: Owner is aware that if the hair on a pet is in a matted condition, the pet may not be able to receive the style of cut requested. If the matting is severe, brushing out may be too stressful and painful to the pet. In this case, SBDG may not be able to continue the groom. A shave down or much shorter cut may be a possible option. We will contact you in this case.
5. Grooming Enclosures: I agree and understand that my dog(s) may be placed in grooming enclosures before, or after grooming.
6. Risks to Pet:
6a. Fuzzy Pet: If my pet is severely tangled or matted, it has a greater risk of injury, stress and trauma. All precautions will be taken. However, problems arise occasionally during or after the grooming process, such as nicks, clipper irritation and mental or physical stress. Owner agrees not to hold SBDG responsible for any injuries, which might result from the grooming process.
6b. Aging and Special Needs Pets: Senior pets (age 7 and over). Owner is aware that as a pet ages, or if the pet has special medical conditions, the process of grooming may become stressful both physically and mentally. The stress of grooming may cause latent unknown, active or inactive conditions to occur. Although SBDG will take responsible care in the grooming of the pet, Owner agrees not to hold SBDG responsible for any reactions to grooming or conditions which stem from the condition of older-aged or ill-conditioned pets.
6c. Moles/skin Lesions/Tumors/Other skin irregularities: Because theses skin irregularities may protrude from the skin, they are vulnerable to nicks or cuts and skin reactions/irritation during the grooming process. This includes saggy skin which is often present in older dogs under arm pits. Owner agrees not to hold SBDG responsible for any of these related injuries.
6d. Pre-existing conditions: Pet owner agrees to hold harmless from damage or loss of claims arising from any known or unknown pre-existing condition. Said pre-existing condition shall include but not be limited to illness, disease, personal injury, rashes, hot spots, medical conditions or ear infections.
7. Grooming Rates: I understand that due to the nature of pet grooming, all quoted prices prior to actually grooming my pet are only good-faith estimates. Final price will depend on the temperament of each pet and the condition of its coat. Pricing can vary from visit to visit. Extra charges may apply for pets that are matted and/or difficult to manage.
8. Allergic Reactions: We offer several formulas of shampoos. All are high quality professional grade level shampoos. Though it is highly infrequent, owners should be aware that it is possible that their dog may have an allergic reaction. It is impossible to know this in advance. If owner has any concern, it is advised that he chooses the HYPO Allergenic shampoo to decrease this chance. Further, owner agrees not to hold SBDG responsible for any subsequent skin infections resulting from any allergic reactions to products used.
9. Picking up of Pet: If your pet is left at SBDG longer than the 30 minutes past completion of grooming, then day-care charges of $10 per half hour may apply.
10. Doing our business: Owner agrees to walk their pet prior to drop off.
11. General Release: By reading, signing and dating this agreement, I hereby waive and release SBDG, its owner, and owners of any or all premises, agents and volunteers of said owner(s), from any and all damages, losses, fines, claims, suits, expenses (including attorney fees and defense costs), judgments and/or liabilities of any form or nature resulting from any act or omission of dog or dog owner participating in pet grooming at SBDG and/or under the auspices of owner. This release of liability includes, but is not limited to any injury, death, sickness or personal injury or property damage my dog or I may suffer while on or around the premises where the pet grooming is held. This release also includes specifically, but without limitation, any and all forms of personal injury (including death) and property damage to myself, other persons, and other animals resulting from the act or omissions of any and all dogs or dog owners and I expressly assume the risk of such dangers or injuries and losses throughout the duration of the pet grooming.
By my signature below, I hereby state that I fully understand and agree to the above terms and conditions in consideration for the pet grooming services provided by SBDG and its employees and/or agents.
